Community Health Navigators

Community Health Navigators (CHNs) help connect individuals to community resources and build individual competence. CHNs have the goal of improving overall community health. We hope to support and educate individuals that can share that information with their communities.

What does a CHN do?

• Help you find resources available in Lenawee County
• Help you set goals for your health
• Support you during your journey toward success 
• Conduct outreach events to promote healthy lifestyles
• Advocate for you to maintain access to a healthy environment

New Year, New Me! The Community Health Navigators will be holding presentations and workshops to encourage Lenawee County to make 2023 their healthiest year yet. The monthly theme and presentation locations are listed below:

Hours and Location

Hours 

We are available Monday – Friday from 8:00 AM to 4:30 PM with a break from 12-1 for lunch.

We are also available at later hours by appointment. Contact us to set up an appointment: 517-264-5226 opt. 2 and ask for a Community Health Navigator.
